ABSTRACT

BackgroundA black female in her 40s presented with a nonpruritic rash for 10 months consisting of bumps on the face, hands, forearms, and thighs. She had no prior treatment. Past medical history was significant for pulmonary embolism (PE) 6 years prior. She had no personal or family history of autoimmune disease. Physical exam revealed numerous smooth 2-3 mm skin-colored papules over the bilateral forearm dorsa, hands, anterior thighs, and face. Serum protein electrophoresis revealed monoclonal IgG lambda gammopathy. Skin biopsy of her left elbow showed dermal fibroplasia with mucin deposition. IgG was less than 1.5 grams/deciliter;bloodwork was otherwise stable. The diagnosis of scleromyxedema was rendered.ObjectivesThe objective of this clinical case was to evaluate a neurologic sequela of COVID-19 infection in a patient with scleromyxedema.MethodsOne month following diagnosis of scleromyxedema, our patient was diagnosed with COVID-19 five days before admission to the emergency department with altered mental status and aphasia. Rheumatology was consulted due to malignant hypertension and acute kidney injury with question of scleroderma-like renal crisis in the setting of recently diagnosed COVID-19 infection, although she had no other features of systemic sclerosis. The infectious disease team was consulted due to COVID-19-induced inflammatory reaction.ResultsThe patient's creatinine kinase and brain natriuretic peptide were elevated. Creatinine and potassium trended upwards. She developed seizures and became hemodynamically unstable with rapidly declining clinical status. She was transferred to the intensive care unit, where she developed respiratory arrest, shock, hyperkalemia, and acidemia. She received escalating doses of pressors but experienced frequent arrhythmic disturbances and developed asystole. Resuscitation efforts were unsuccessful;she expired within 24 hours of consultation.ConclusionDermato-neuro syndrome (DNS) is a potential complication of scleromyxedema associated with confusion, dysarthria, seizures, and coma. The patient's clinical presentation is consistent with DNS in the setting of scleromyxedema likely precipitated by COVID-19. Intravenous immunoglobulins are first-line treatment for scleromyxedema;however, it is associated with risk of venous thromboembolism. The patient was considered for treatment as an outpatient but deferred due to history of PE. She was reevaluated for treatment upon presentation to the hospital, but given the severity and rapidity of her condition, it was already too late. This is the second reported case of COVID-19 induced DNS in a patient with scleromyxedema. ABSTRACT

Hotels have increasingly engaged in environmentally responsible initiatives to demonstrate their commitment to environmental concerns and sustainable hospitality and tourism. These initiatives are expected to become even more popular in the context that the COVID-19 crisis has driven people to further acknowledge the importance of the ecosystem. This study aims to examine how hotels' environmental corporate social responsibility (CSR) affects customers' green word-of-mouth (WOM). Structural equation modeling was employed to analyze data from an online survey of 749 Chinese respondents. The findings reveal that hotels' environmental CSR indirectly enhances customers' green WOM intention via green perceived value (i.e., cognitive route) and green hotel pride (i.e., emotional route). Furthermore, the indirect effects of hotels' environmental CSR on customers' green WOM are more substantial for hotels with higher star ratings. These findings offer valuable insights for hoteliers to develop genuine environmentally responsible initiatives that can generate positive customer responses.

ABSTRACT

Introduction: Drug-induced liver injury (DILI) is defined as hepatic dysfunction caused by prescription medications, supplements, or xenobiotics after alternative causes have been excluded. As one of the leading causes of acute liver failure, DILI should be considered when patients present with hepatic dysfunction. We present a case of symptomatic DILI secondary to artemisinin use. Case Description/Methods: A 78-year-old Chinese man with no medical history presented to the hepatology clinic with 10 weeks of jaundice, weakness, and pruritis. He started taking Artemisinin/ Bioperine 12 weeks ago to prevent COVID-19 but stopped 3 weeks ago. He denied abdominal pain, a family history of liver disease, substance/alcohol use, and taking other concomitant drugs. Physical examination revealed scleral icterus and no other signs of chronic liver disease. Laboratory studies showed total bilirubin 11 mg/dL, alkaline phosphatase 293 U/L, aspartate transaminase 170 U/L, and alanine transaminase 196 U/L with negative workup for hepatitis A, B, and C. CT abdomen and MRCP were unremarkable for liver or biliary pathology. Further serological workup was negative and follow-up labs revealed normalization of liver enzymes and bilirubin. Given the patient's improvement, liver biopsy was not pursued. The patient was instructed to avoid supplements unless prescribed by a physician. Discussion(s): DILI is a global issue with an estimated annual incidence rate of 13.9 to 24.0 per 100,000 persons. Diagnosing DILI is important as it can cause acute liver injury and liver failure in certain cases. Since COVID-19 emerged, supplement use has increased given claims of boosting the immune system. Artemisinin is an herb used in traditional Chinese medicine with antimalarial activity investigated to be a possible COVID-19 treatment, but no current evidence exists to support it being effective against COVID-193. Our patient's supplement also contained Bioperine, a black pepper extract, which is likely benign. Contrarily, artemisinin is a well-described cause of idiosyncratic acute liver injury and hepatotoxicity, causing self-limited mild to moderate transaminitis but also severe cases requiring emergent livertransplantation. Our patient's unrevealing workup, his spontaneous improvement correlating with supplement discontinuation, and RUCAM score of 7 led to high suspicion of DILI secondary to artemisinin. Providers should always ask patients about supplement use and consider DILI when patients present with liver injury. (Table Presented).

ABSTRACT

Acquired von Willebrand syndrome (AVWS) contributes to bleeding during extracorporeal membrane oxygenation (ECMO) support. Although it is recognized that AVWS rapidly resolves after ECMO decannulation, this approach may often be clinically unsuitable. In such cases, optimal AVWS management during ECMO support is not well established. We report our approach to managing AVWS in a patient on veno-venous (VV) ECMO for 59 days. A 19-year-old male developed hypoxemic respiratory failure from SARS-CoV-2 pneumonia. Following intubation, he progressed to VV-ECMO support for refractory hypoxemia and was started on bivalirudin for systemic anticoagulation. Two days later, he developed refractory gastrointestinal and oro-nasopharyngeal bleeding despite blood product transfusions and discontinuing bivalirudin. He was started on pantoprazole along with infusions of octreotide and aminocaproic acid. Upper endoscopy on ECMO day 5 revealed an ulcerative bleeding vessel in the duodenum that was clipped. Recurrent mucosal bleeding precluded resumption of systemic anticoagulation. On ECMO day 23, AVWS was diagnosed based on elevated von Willebrand factor (VWF) activity (207%, normal 55-189%) and antigen (234%, normal 50-210%) levels with abnormally low VWF high-molecular-weight multimers. Factor VIII complex was administered twice over the following week. Between doses, the ECMO circuit was exchanged to empirically mitigate suspected shear-related VWF consumption from the fibrin burden, and a repeat endoscopy controlled additional intestinal bleeding with local hemostatic agents. He received 36 units of red blood cells, 2 units of platelets, 2 units of plasma, and 7 pooled units of cryoprecipitate over 31 days leading into these combined interventions. In the 28 days afterwards, he received 3 units of red blood cells, 3.5 pooled units of cryoprecipitate, and no additional platelets or plasma. Our patient was maintained off systemic anticoagulation for 54 of 59 days of VV-ECMO support without any thrombotic complications occurring. With no subsequent clinical evidence of bleeding, repeat VWF testing was done two months post-decannulation and showed near-normal VWF activity (54%) and normal multimer distribution. Our patient rehabilitated well without any neurologic deficits and on discharge was requiring supplemental oxygen with sleep and strenuous activity. Avoiding systemic anticoagulation, repleting VWF, maintaining circuit integrity, and providing local hemostasis, when possible, may be a safe and effective management strategy of AVWS on ECMO support when decannulation is not a viable option.

ABSTRACT

Background: Since early 2020, the novel SARS-CoV-2 virus has spread rapidly throughout the globe. Subsequently many individuals have developed some form of immunity due to either a prior infection, one or more vaccinations, or a combination of the two. Using local epidemic data and mathematical modeling, we enumerate the various immune populations in Washington State and Oregon and quantify the level of protection against infection and hospitalization. Method(s): We developed a compartmental model of ordinary differential equations, which stratifies the population by age (0-17 years, 18-49 years, 50-64 years, and 65+ years), region, type of immunity (naive, infectionderived, vaccine-derived, booster-derived, hybrid immunity, etc), and recency of immune conferring event (recent and waned). To track the number of individuals in each category we combine 1) literature-based estimates of susceptibility to infection and severe disease by age, immune status, and variant, 2) calibration to the number of severe infections (hospitalizations and deaths) and number of vaccinations and 3) validation with serological surveys of the population. Result(s): We estimate that by mid-April 2022 more than 95% of the populations of both Washington and Oregon had some immunity against COVID-19 infection and hospitalization. Younger age groups tended to have much higher rates of natural or hybrid immunity with 96% of 0-17-year-olds and 83% of 18-49-year-olds protected due to past infections. Overall, the population-level immunity against the Omicron variant reduced risk of infection by 59% (95% Credible Interval 54% - 62%) and risk of hospitalization by 79% (95% CI 77% - 81%) in Washington and 62% (95% CI 57% -66%) and 83% (95% CI 82% - 85%), respectively, in Oregon. There was similar population-level protection against Delta at the start of the Omicron wave in early December 2021, which reduced risk of infection by 60% (95% CI 56% - 63%) and risk of hospitalization 79% (95% CI 78% - 80%) in Washington and 66% (95%CI 63% - 70%) and 82% (81% - 83%), respectively, in Oregon. Conclusion(s): Very large waves of new infections throughout 2021 and early 2022, in addition to high levels of vaccination and boosting among the older age groups in Washington and Oregon have greatly reduced population susceptibility to currently circulating strains. However even very high population immunity has allowed for emergence of novel variants that escape existing immunity, highlighting the need for continued develop of new variantspecific boosters.

ABSTRACT

Background: Autophagy, a cytosolic-structure degradation pathway, allows production of IL21 by CD4 T-cells and efficient cytolytic responses by CD8 T-cells. Autophagy is in part regulated by acyl-CoA-binding protein (ACBP) which has two functions. Intracellular ACBP favors autophagy, whereas secreted extracellular ACBP inhibits autophagy. Herein, we assessed whether autophagy and the ACBP pathway were associated with COVID-19 severity. Method(s): Through the BQC-19 Quebec biobank, somalogic proteomic analysis was performed on 5200 proteins in plasma samples collected between March 2020 and December 2021. Plasma from 903 patients (all data available) during the acute phase of COVID-19 were assessed. COVID-19 severity was stratified using WHO criteria. In vitro, ACBP intracellular levels, autophagy levels (LC3II) and IL21 production were assessed by flow in PBMCs after a 24h stimulation with IL6, phorbol myristate acetate (PMA)+ionomycin or lipopolysaccharide (LPS). Plasma levels of anti-SARS-CoV-2 (full spike protein or RBD) IgG were assessed by ELISA. Result(s): Median age of the cohort was 62 yo, 48% were female, 55% had comorbidities (see table). Increasing plasma levels of ACBP were found with severity (mild, moderate, severe and fatal groups having 5.3, 7.3, 9.5 and 10.6 RFU/50muL of plasma, respectively, p< 0.001 for all comparisons). Patients with comorbidities had higher plasma ACBP levels (7.4 vs 6.4 RFU/50muL, p< 0.001). Plasma ACBP levels were higher during the delta and omicron-variant periods (8.4 vs 6.8 RFU/50muL;p< 0.001). Plasma ACBP levels correlated with LC3II levels (r=0.51, P< 0.001) and IL6 (r=0.41, p< 0.001), but neither with markers IL1beta nor IL8. ACBP levels negatively correlated with IL21 levels (r=-0.27, p< 0.001), independently of age, sex, and severity. ACBP levels were not associated with levels of anti-SARS-CoV-2 IgG levels. In vitro, IL6 stimulation of healthy control PBMC induced extracellular ACBP release. Moreover, adding recombinant ACBP: 1) reduced autophagy in lymphocytes and monocytes upon polyclonal stimulation with PMA/ionomycin or LPS;2) reduced intracellular production of IL21 in T-cells after PMA/ ionomycin stimulation. Conclusion(s): Plasma ACBP levels were inversely linked with IL21 levels, suggesting that autophagy and IL21 allow control of SARS-CoV-2 infection, independently of the level of SARS-CoV-2 antibody secretion. ACBP is a targetable autophagy checkpoint and its extracellular inhibition may improve SARS-CoV-2 immune control. (Table Presented).

ABSTRACT

Purpose of Study: Since 1972, Bhutan has followed the philosophy of Gross National Happiness (GNH) - an alternative to GDP which quantifies the holistic wellness of its citizens' as opposed to the production of materials as a metric of the nation's development & prosperity. Nonetheless, the government identified youth and young adult (YYA) mental health to be a major/increasing point of concern from a 2015 national survey, indicating need for a closer look at Bhutan's mental health burden. By exploring YYA perspectives of mental health via an online survey, we hope to inform resource creation, support existing infrastructure, & guide future research. Methods Used: Survey questions were developed via collaboration of Canadian and Bhutanese shareholders to ensure cultural appropriateness and relevance. In all, 11 demographics questions, 17 10-point Likert scale questions, and 7 written answer questions were included in the survey. Students enrolled at university in Thimphu, Bhutan, between the ages of 18-25 inclusive and who are Bhutanese citizens or who have lived the majority of their life in the country were recruited via email. 289 participants consented to & completed the online survey via Qualtrics. Survey responses were quantitatively analyzed in R-Studio, save for written responses still under qualitative analysis in NVivo and not included in this discussion. Summary of Results: On average, YYA indicated feeling sad, anxious, and/or depressed more frequently than they perceived peers/family to feel the same. Strong agreement was seen regarding treatability of mental health conditions (median of 9 on 10-point Likert scale). Environmental factors, adverse childhood experiences, & personal weakness were noted to have detrimental impacts on wellbeing. The majority of respondents indicated positive familial relationships & confidence in parents to understand/support mental health issues, with medians for all family-related 10-point Likert scale questions being above 7.5. Most respondents agreed they were able to get through difficult times and be self-reliant, with all associated 10-point Likert questions having a median over 5 for moderate agreement. A general positive response to using apps or websites as mental health resources was noted. Relating to COVID-19, minimal negative outcomes in terms of diet, anxiety/depression, home life, & substance use were reported. Strong agreement (median of over 7.5 on 10-point Likert scale) was noted with regard to increases in domestic violence as a result of the pandemic. Conclusion(s): Differences between self-ratings and perceptions of others' mental health by Bhutanese YYA indicate a lack of open communication about the topic. Directing resources towards discussions about mental health may be beneficial - especially if technologically accessible. Impacts of the pandemic may continue to develop as Bhutan emerges from lockdown protocols;increasing domestic violence is a primary concern of Bhutanese YYA meriting research.

An important variable in measuring potential access is the competitive nature of the service being provided. Health services such as physician visits or hospital beds are often viewed as rival goods where one's consumption of a service will diminish another's ability to consume the same service. Congestion at facilities is as important as the overall level of supply at facilities. For rival goods, accessibility and congestion are linked as reciprocal concepts. The relationship between accessibility and congestion is even more important in the current era of the COVID-19 pandemic because critical care services, such as the need for intensive care unit beds, illustrate the need to focus on a balanced level of congestion among hospitals to prevent care failure at the local level. This research investigates the role of service congestion in various existing and proposed models and measures using data from the state of Illinois. Because evenness of service congestion and lower travel times are conflicting goals, evenness of congestion as measured by Gini coefficients is weighed against the cost of travel to determine a compromise solution. Results suggest that the rational agent access model and the congested supply accessibility model provide such compromises when used in conjunction with the transportation problem.

ABSTRACT

This article introduced a new deep learning framework for fault diagnosis in electrical power systems. The framework integrates the convolution neural network and different regression models to visually identify which faults have occurred in electric power systems. The approach includes three main steps: 1) data preparation;2) object detection;and 3) hyperparameter optimization. Inspired by deep learning and evolutionary computation (EC) techniques, different strategies have been proposed in each step of the process. In addition, we propose a new hyperparameters optimization model based on EC that can be used to tune parameters of our deep learning framework. In the validation of the framework's usefulness, experimental evaluation is executed using the well known and challenging VOC 2012, the COCO data sets, and the large NESTA 162-bus system. The results show that our proposed approach significantly outperforms most of the existing solutions in terms of runtime and accuracy.

ABSTRACT

Internet-delivered psychological treatments (IDPT) consider mental problems based on Internet interaction. With such increased interaction because of the COVID-19 pandemic, more online tools have been widely used to provide evidence-based mental health services. This increase helps cover more population by using fewer resources for mental health treatments. Adaptivity and customization for the remedy routine can help solve mental health issues quickly. In this research, we propose a fuzzy contrast-based model that uses an attention network for positional weighted words and classifies mental patient authored text into distinct symptoms. After that, the trained embedding is used to label mental data. Then the attention network expands its lexicons to adapt to the usage of transfer learning techniques. The proposed model uses similarity and contrast sets to classify the weighted attention words. The fuzzy model then uses the sets to classify the mental health data into distinct classes. Our method is compared with non-embedding and traditional techniques to demonstrate the proposed model. From the experiments, the feature vector can achieve a high ROC curve of 0.82 with problems associated with nine symptoms.

This paper constructs macroeconomic uncertainty indices for China from 2009 to 2021 based on the distribution of forecast errors of major macroeconomic variables. The indices effectively capture the economy's deep slump and rebound in 2020. This study reveals a negative relationship between macroeconomic uncertainty and firms' investment, with downside uncertainty having a larger negative impact than upside uncertainty. © 2023 Elsevier B.V.

ABSTRACT

The outbreak of COVID-19 reminds us of the urgency of reducing wild meat consumption (WMC). It is important to investigate why some people prefer wild meat. Using in-depth interviews (n = 19), group interviews (n = 9), and participant observation, this study conducted fieldwork in Nanxiong in South China in 2015–2016 and 2020, where WMC is considered a historical and cultural tradition. This study updates understandings of people's need for WMC in recent years and is likely the first attempt to apply social practice theory to investigate WMC, which provides an integrated and promising tool for theoretically summarizing the "motivations” in everyday life and dynamic relations behind this collective and conventional consumption. This paper offers new understandings of wild meat consumption through an exploration of how the related practices of preparing, cooking and tasting are changing. It explores materiality of taste and texture and the agentic capacity of wild meat. Additionally, the concept of practice memory is used to examine the development of WMC practice. In the context of COVID-19, some policy implications against WMC are provided. © 2023 Association for the Study of Food and Society (ASFS).

ABSTRACT

Objective: To develop a SARS-CoV-2 antigen detection management system for Chinese residents under community grid management, which is supported by "health information technology" and "neural network image recognition", so as to give full play to the advantages of "grid management". This system is applied to the normalized prevention and control of COVID-19 epidemic. Method(s): The model of image recognition algorithm was built based on deep learning and convolution neural network (CNN) artificial intelligence algorithm. The improved Canny edge detection algorithm was used to monitor and locate the image edge, and then the image segmentation and judgment value calculation were completed according to projection method. The system construction was completed combing with the grid number design. Result(s): The proposed method had been tested and showed the accuracy of the algorithm. With a certain robustness, the algorithm error was proved to be small. Based on the image recognition algorithm model, the development of SARS-CoV-2 antigen detection management system covering user login, paper-strip test image upload, paper-strip test management, grid management, grid warning and regional traffic management was completed. Conclusion(s): Antigen detection is an important supplementary means of COVID-19 epidemic prevention and control in the new stage. The SARS-CoV-2 antigen detection management system for Chinese residents under community grid managemen based on image recognition enables mobile communication devices to recognize the image of SARS-CoV-2 antigen detection results, which is helpful to form a grid management mode for the epidemic and improve the management framework of epidemic monitoring, detection, early warning and prevention and control.Copyright © 2023 The Authors

ABSTRACT

To combat the spread of the COVID-19 virus, countries enforced quarantines, physical and social restrictions on people. These restrictions left many feeling isolated and lonely due to prolonged quarantines and lockdowns. This raises questions about using robots as social support to alleviate these symptoms, while still complying with restrictions and regulations. Since acceptance of social robots as companions has traditionally been low, an event like COVID-19 could change acceptance of robots as social companions as loneliness can influence the likelihood of anthropomorphizing nonhuman agents. Here, we aimed to see if loneliness, due to COVID-19 restrictions, influence the Uncanny Valley pattern that prior work has shown. As such, participants saw robot images that varied in physical human-likeness and were asked to evaluate them regarding trustworthiness, mind perception and likability. The measurements were obtained once before COVID-19 (in 2016) and once at the peak of the pandemic in September 2020. Results show that ratings of mind perception and likability were significantly impacted by the pandemic, with less pronounced UV patterns for those who experienced the COVID-19 pandemic. However, no differences in the UV pattern was observed on trust. Post-hoc analyses also illustrated that people were more likely to judge machinelike robots negatively, which could be due to increased loneliness/anxiety. These data suggest that loneliness attenuates UV patterns that are observed in "Uncanny” robots and that people have more favorable attitudes towards humanlike robots when feeling lonely, which provides important considerations for the use of humanlike robots as social companions. © 2022, The Author(s), under exclusive license to Springer Nature Switzerland AG.

ABSTRACT

In the United States, public transit vehicles have a very low average load factor (10.1-12.4%), resulting in an excessive waste of seat capacity and poor fuel economy per passenger mile served. This problem is gravely exacerbated by the COVID-19 pandemic, which at its peak had caused more than 70% reduction in transit ridership nationwide. On the other hand, the rapid uptake of e-commerce, also accelerated by the pandemic, has put tremendous pressure on last-mile delivery. A co-modality system that integrates transit services with last-mile logistics offers a promising solution to better utilization/sharing of vehicle capacity and supporting infrastructure. Here we show such a system may be implemented based on Autonomous Modular Vehicle Technology (AMVT). At the core of AMVT is the ability to operate a fleet of modular autonomous vehicles or pods that can be moved, stationed, joined, and separated in real time. Coupling modularity with autonomy is poised to enable co-modality and beyond. We describe an AMVT bimodality system that provides integrated public transit and last-mile logistics services with a fleet of pods and discuss relevant research challenges and opportunities, research approaches, and real-world adoption issues. © 2009-2012 IEEE.
